Summary:

Breast Care Patient Navigator Non-Registered Nurse serves as an essential part of the breast care team by supporting timely patient care and follow up. This job is an advocate for the patient in their dealings with the healthcare community, helping patients navigate their way to timely and quality care and assisting the breast care team in patient care initiatives and programs.

Responsibilities:

  • Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
  • Help patients navigate their way to timely and quality care from suspicious findings to follow-up testing, while addressing tangible barriers to patient care.
  • Facilitate appointments for consults and support services within established standards.
  • Function as a member of an interdisciplinary team to assess, secure and coordinate appropriate services.
  • Help qualified patients secure services from any available grants from state and or national programs.
  • Work with Breast Care Team and others to collect data, track outcomes and support strategic work plans.
  • Participate in educational outreach efforts to community and to key customers (physicians, office staff, diagnostic staff, etc.) to establish and maintain positive working relationships.
  • Perform all other duties as assigned.

Requirements:

  • High School diploma or equivalent required.
  • Greater than 1 year of healthcare experience.
  • Previous hospital experience is a plus.
