About your Role:

Our Intake Coordinator serves a vital role as our first impression with clients and referral sources in the community. You will meet with adult potential clients to document their history, strengths and weaknesses, goals, and support systems, to create a comprehensive intake assessment of their service needs. Using this treatment plan, you will link the client to necessary services within our team. This role works remotely from home, in the office, and meets with clients in the community to screen potential referrals, conduct Intake assessments, and complete authorizations. You will represent Family Preservation Services and our programs by maintaining a positive and professional working relationship with referral sources and community stakeholders.

Does the following apply to you?

  • LPC, LCSW, or LMFT licensed in Louisiana
  • Experience providing services to adults with severe mental illness
  • Valid driver's license in good standing
